Forms

National Insurance Institute forms for submitting claims for benefits, insurance and collection issues are available here.

The forms are in PDF format. For viewing or printing them you need Acrobat Reader, available for free on the internet. To download Acrobat Reader and for guidelines, click here.

The forms may be submitted in the following ways:

• Through the National Insurance Institute website (in Hebrew)
• By filling out the forms and submitting them online. If you hold a biometric ID, it is important to activate the card before completing the form. Information and instructions regarding ID activation.
• By mailing to the relevant branch
• By faxing to the department. Fax numbers can be found by clicking on the relevant cities as they appear in the list of branches and then the Department Details tab (in Hebrew).
• By placing them in a service box of the local branch.
