Power of attorney


Every representative must submit a power of attorney signed by the insured before he is allowed to perform actions in the insured's account or receive any information from the National Insurance.

Representative connected to the client representation system of the National Insurance

Shall fill out an authorization of power of attorney online on the website.

Representative not connected to the client representation system 

Shall fill out and send power of attorney form signed by the insured to the National Insurance branch handling the client's file.
This form may also be sent through the document delivery service on the website.

Please note that at the end of the form there is a paragraph concerning information on benefits. The representative will be allowed to receive information concerning benefits only if the client signs this part of the form as well.
