The Claim for Determining Degree of Work Disability form should be submitted to the National Insurance Institute branch closest to the place of residence. The claim can also be submitted by mail, by fax, or in the service box of the branch.
An insured person for whom a degree of work disability has been established (even 0%) is no longer entitled to an injury allowance for that injury.
The claim should be submitted within 12 months from the date of the injury. Submission of the claim after that time may adversely affect entitlement to the benefit, in whole or in part. It is best to submit the claim for determining the degree of work disability as soon as possible after the end of the period of payment of the injury allowance.
