Public Inquiries

Much information can be found in the website regarding your rights and obligations in the National Insurance Institute. There are a variety of ways to contact the National Insurance Institute:

 

 For complaints and personal appeals, use the Claim Form. Please make sure to note your ID number on the form.

 

 For general questions, use the General Questions Form , and the professional  department  will respond as soon as possible. Due to the Personal Privacy Law, you are requested not to enclose any personal medical information. For more information (personal information) you can call the National Call Center .

 

 For addresses, telephone numbers and email addresses of local branches and the different service channels, please click here.

 

 The National Call Center provides the followings services: general information, inquiries – personal information, ordering authorizations, debt payments using credit cards, ordering payment stubs, ordering blank forms and guidebooks.

 

 To receive information about National Insurance Institute activities, or to receive an application for public information according to the Freedom of Information Law (obtaining information according to this law entails a token payment), please contact the person in charge of freedom of information.
